Assumption of Risk: Participation in Canyon Academy FC activities involves risks including physical injury, collisions, falls, and other athletic hazards.

Release of Liability: I release Canyon Academy FC, its coaches, staff, volunteers, and affiliates from liability for injuries sustained during participation.

Medical Authorization: I authorize Canyon Academy FC representatives to obtain emergency medical care for the participant if necessary.

Code of Conduct: Players must respect teammates, coaches, referees, staff, and club policies.

Media Release: Photos and video taken during camp activities may be used for club promotion.
